Why choose to vaccinate against RSV?

RSV is a common respiratory virus that infects almost all Canadians by the age of 2 years and causes recurrent infections throughout their life.2 Most adults experience symptoms similar to a common cold. However, for some individuals – particularly those with increasing age – RSV infection can lead to severe disease.3

RSV activity peaks during the fall and winter months and can have serious complications for older adults and adults with certain medical conditions.4,5

The National Advisory Committee on Immunization (NACI) strongly recommends vaccination against RSV for all adults 75 years and over, and for certain adults at high risk of severe RSV disease.2

Identifying patients at increased risk for severe RSV disease2

While RSV generally presents as a mild illness, some populations are at increased risk for severe RSV disease and outcomes.2,6 And for those also living with chronic comorbidities, the risk may be even higher.2

CARDIORESPIRATORY DISEASE

  • Includes: cardiac or pulmonary disorders (e.g., COPD, asthma, cystic fibrosis, conditions affecting ability to clear airway secretions)2

METABOLIC DISEASES

  • Includes: Class 3 obesity (BMI ≥40 kg/m2); diabetes mellitus; other metabolic diseases2

IMMUNOCOMPROMISED

  • Includes: moderate and severe primary immunodeficiency diseases; solid organ or blood stem cell transplant; immunosuppressing medications (e.g., corticosteroids)2,7

NEUROLOGIC CONDITIONS

  • Includes: neuromuscular, neurovascular, neurodegenerative [e.g., dementia], neurodevelopmental conditions; seizure disorders2†

OTHER DISEASES

  • Includes: chronic kidney disease; chronic liver disease2

NACI guidance on RSV vaccines for older adults and those at high risk for severe disease2‡

STRONG recommendation2

Vaccine immunization programs should include:

All adults aged ≥75 years
Adults aged 65–74 years who are at increased risk for severe disease
due to significant chronic health conditions
Adults aged ≥18 years who:
  • are residents of nursing homes and other chronic care facilities
  • have had a lung transplant
  • have had a hematopoietic stem cell transplant (in the previous two years or who remain on immunosuppression)
  • are on home oxygen or require chronic oxygen therapy regardless of living at home or elsewhere
  • are receiving dialysis

Considerations:

There is still uncertainty about the total duration of protection and the need for and timing of any potential booster doses with any of the authorized adult RSV vaccine products. Therefore, a single dose is currently recommended.

DISCRETIONARY recommendation2

Recommendation for individual decision-making (i.e., clinicians wishing to advise individual patients about immunization against RSV):

RSV vaccines may be considered by adults aged 18–64 years at increased risk for severe RSV disease

Considerations:

  • A careful assessment of benefits vs. potential harms is warranted, as most younger adults may benefit more from vaccination at an older age or if their risk for severe RSV disease increases.
  • A single dose of vaccine is currently recommended and is expected to provide protection for RSV disease for ≥3 years. For adults aged <60 years at increased risk, the currently recommended dose should be administered at a time when maximal clinical benefit is expected.

Indications and clinical use:

mRESVIA® (Respiratory Syncytial Virus [RSV] mRNA Vaccine) is indicated for active immunization for the prevention of lower respiratory tract disease (LRTD) caused by RSV in adults 60 years of age and older, and adults 18 through 59 years of age who are at increased risk for LRTD caused by RSV.

Pediatrics: safety and efficacy in individuals under 18 years of age have not been established. mRESVIA is not indicated for pediatric use.

Contraindications:

  • Hypersensitivity to mRESVIA or to any ingredient in the formulation, including any non-medicinal ingredient, or component of the container.
  • Children less than 2 years of age due to an increased risk of severe and/or life-threatening RSV-enhanced respiratory disease reported in infants during a pediatric study. The risk of RSV-enhanced respiratory disease is not applicable to adults.

Relevant warnings and precautions:

  • Postpone vaccination in individuals with an acute severe febrile illness
  • Syncope
  • Anaphylaxis
  • Use in individuals with thrombocytopenia or coagulation disorders
  • mRESVIA may not protect all vaccine recipients
  • Use in immunocompromised individuals
  • Not recommended during pregnancy
  • Use in breastfeeding individuals
